If you have not found the information you are looking for in the publications and pages on our website, please follow the instructions below to submit a specific request for information from IR.

 

• If you are on campus and have access to the my.calpoly.edu portal, please follow the SRS Ticket Instructions (PDF) to submit an SRS ticket (Technical Service Request) to IR. Once your Technical Service Request is submitted, IR will receive an automatic e-mail notification of your request.

• If you are not on campus and do not have access to the my.calpoly.edu portal, please complete the IR Information Request (PDF) form.
